AzureでSQL Serverを使用しています。具体的には、価格設定/モデル層「S4Estándar(200 DTU)」のAzure SQL Database。
大量のデータを受信していますが、インターネット接続はボトルネックではないと思います。なぜなら、他のクエリでも大量のデータを受信し、より高速だからです。
また、このテーブルに大量のデータを挿入しています。数日で、ファイルテーブルに最大2億4000万行(1 cidの場合)、値に4億8千万行のテーブルが追加されます。

統計は最新です。年およびその他の列のデータ型を変更しましたが、パフォーマンスは少し良くなったようですが、実行計画は同じです。カーディナリティの推定(インデックスの変更)を修正しようとしていますが、まだ成功していません。Azureのドキュメントによると、データベースには130の互換性レベルが必要ですが、ProductVersion 12.0では既に100になっています。
